How can I form a poem from these sentences: "I want to make her smile like that. I want to be the one. I want to be the reason."?

If I understand you correctly, you want these words to be turned into a poem? My first suggestion is to advise you that poems do not have to rhyme.

Then I would advise you to try rewriting the sentences adding more detail and description. Create a scene for your audience to visualize. Then decide what image you want your readers to have at the end of each line. Put your line breaks there.

For example.:

Through the frosted window,

he watches.

She smiles at her companion,

Her lips curve gently

Just as the used to for him

And her eyes shine

Eclipsing the brilliant crystal glow

Of the chandelier.

Her companion rises,

And leaves her unguarded.

Afraid to go in,

Yet too captivated to move on,

The watcher thinks,

I want to make her smile like that.

I want to be the one.

I want to be the reason.

Hope this helps.
