Why is the nurse upset with Juliet? Does she approve of her relationship with Romeo?

The nurse is very upset with Juliet, but less because of her love for Romeo and more, at first, because of the fact that she does not seem to want to go along with the plan of her marrying the great Prince Paris.  Because the nurse is loyal to the family, as well as to Juliet, she wants to please Juliet's mother and the family by helping to facilitate the marriage with Paris.

Once she starts to understand how completely crazy she is for Romeo, she begins to change her tune and by the end has completely changed sides and is working to unite Romeo and Juliet and trying her best to make sure they can get married, help them exchange messages, etc.  So in the end she completely approves.
