The last play to be written by William Shakespeare and John Fletcher was, "The Two Noble Kinsmen" It was published in 1634. However, there were links between "The Two Noble Kinsmen" and contemporaneous works which place the date of authorship and performance at around 1613-14.

The Globe theatre burned down in June 1613 during a performance of Henry VIII. And many think of this as Shakespear's last true play.  It was performed in June of 1613 so "The Two Noble Kinsmen" was the only play to be written for and performed in the "Blackfriars, the smaller indoor theatre that the King's Men had first occupied in 1609."

It is commonly accepted that the last play Shakespeare wrote alone was The Tempest. This was completed sometime between 1610 and 1611. I am not able to find any information on a tragedy that occured during a performance of this particular play; however, during a performance of Henry VIII the Globe Theater caught fire and burned. The fire occured on June 29, 1613 and was caused by cannon fire used as a special effect.
