As this novel opens, the narrator describes his delight at finding an inn with decent food off the MI (the English equivalent of an interstate). That inn is called the Green Man, and it is in the village of Fareham in Hertsfordshire (Herts).
The other abbreviated terms are from the inn restaurant's menu, which the narrator describes almost word for word. I don't know what they all mean, but in the sources section I've pasted some links where you should be able to find all of them.
