I can suggest a couple of approaches that you might find useful.
In an interview just before she died, Flannery O'Connor said that, in her stories, she tried to describe "an action of grace in territory held by the devil." Taking that comment as your starting point, it would be worthwhile to examine the grandmother and her relation to The Misfit. An interesting essay could be written on whether or not there is an "action of grace," on the part of the grandmother, in "territory held by the devil," in this case, The Misfit. For most of the story, for example, the grandmother is depicted as a rather self-absorbed, shallow person, but there is a point at the end, when she sees how genuinely troubled The Misfit really is, when she steps outside of her limited self for a moment. I think that's the "action of grace," and it would provide a good theme for an essay.
A second approach--the analysis of a symbol--would be to discuss what clothes and manners appear to mean to the grandmother and contrast that with what those things really mean in the context of the story. In other words, the grandmother uses clothes and manners as the measure of a person's worth--how does that actually work out when she is confronted by The Misfit?
