Why not inject some humor into the title by using one of the traits as part of the title? For instance, people with ADHD often procrastinate, or they spend too much time on an insignificant task while they neglect the important or critical issues that need to be addressed.

So, perhaps, one title could be
"In Procrastination Mode." "Too Nervous to Entitle"

Also, there is a certain impulsiveness that is demonstrated in those who have Attention Deficit Disorder. So, you could write one title, draw a line through it, and then write a second one after it. For example, use one of your main ideas from the poem for a title, then, change to another idea. Perhaps, you have something that describes how a person feels with this condition at one moment and then how he/she feels at another. For example,

"What Is This Energy, Energy? O, Splendid Butterfly O'erhead" (here poetic spelling is used)
