First, I will clarify and neaten things up for you a bit.

"Oh Lord, you know me, and you know my strengths and my weaknesses. Please help me make the most of those strengths I possess and direct me in ways that I may correct and transform my weaknesses. Give me insight that I may better appreciate the love that I am so fortunate to share with so many, and help me find a sense of direction in my life that uses the talents you have so graciously bestowed upon me in the most positive, loving and supportive ways."

Given this beginning prayer, you can simply expand it. Talk about your strengths and weaknesses. Be specific about your good and useful traits as well as those things that hamper you from doing your best. Tell how you plan to optimize your strengths and what you might do to confront your weaknesses. Be as honest and straight forward as you can be.

Now, what would God write back? First, He would thank you for coming to him in all your humble sincerity. He would say how much he understands and appreciates your request. Then he would tell you that the answers to all of your questions can be found within you because that is where God resides. It is you who are responsible for the work you do to improve yourself. And the best time to get started is the ever-present now.

The prayer at the end need only restate what you have already asked for in the beginning but with the added understanding that you alone are responsible for the things you do and the positive choices you make.

God is able to witnesses each and every thing that a person does right or wrong.  Because a person is human they are going to make mistakes and demonstrate weaknesses.  In the bible weaknesses are thought of as sin.  The person making the statement is asking God to help him to know how to go forward without continuing to make the same mistakes.

In the second part of the prayer, the person is asking God for Him to help them to know that what he has and who he has in his life are good, because so often people complain about their lives and their loved ones.  They often take their loved ones for granted.  The person in the prayer is asking God not to let him do that.  Instead he wants to be grateful for everyone in his life who he loves and who loves him.

In the last part the person is asking God to help him to become all that he can in God's favor.  He knows that God has given him the gift of talents.  He also knows that he should use his gifts to serve God.  He is asking God to guide him so that he may honor God by using his talents to please him.
