One topic of great concern to the HCFA is medication errors. Current reporting of medication errors is voluntary and, since medication errors incur a loss of reputation and additional paperwork to file, medication errors are believed to be dramatically under-reported. The costs of insurance claims for known medication errors in which medications were omitted or delayed was $198,000 annually; for errors in ordering in which medications were not ordered or were ordered in the wrong quantities, $122,700; for adverse reactions to medications, $88,000. Consumers need the empowerment of having accurate information on medication errors even though current HCFA medication error reporting is voluntary and even though some organizations have begun to call medication errors "therapeutic misadventures.".
