Ok, so here's my scenario: Mollycoddle Hair and Spa, based in Antigo WI, is opening a new store further south in Green Bay. Mollycoddle is the only Hair and Spa place in the area and is quite successful, but looking to expand and maybe even franchise their business. So, i need to conduct a survey of 23 questions consisting of force fed questions, open ended questions, rating scale questions, level of agreement questions, and five general demographic questions. I've came up with several questions already, but I really really could use many more ideas for questions that I could do. Please list as many useful questions as u can think of. Thanks. :)

What services did you purchase? How satisfied were you with the quality of each service? How satisfied were you with the price of the service? What services are not offered that you would like to see offered? Was your visit part of your regular maintenence or did you visit because of a special occasion? If yes, what was the occasion? Did you redeem a gift card? Which of the following would make you more likely to visit/continue to visit: (make a list of things like, monthly specials, offering coupons, gift with purchaces, discounts on products with service, etc.) How likely would you be to recommend mollycoddle to a friend?
