Well, this is a very interesting question. First, let's deal with the format--what a Power Point document should look like. I think it would be perfectly fine to use bullet points, since this is the format most presentations like these follow. So, just keywords or phrases will be fine, as long as they make your meaning clear.
Now, let's get to the content. The first thing to keep in mind is that you will present your information in a sequence of events that took place in the past. To make this easier, you can create a timeline.
To do this, simply draw a line on a piece of paper. Along this line, make notes of the various events in your life, starting with your birth. Where did you start school? What primary school did you attend? What important things happened in your life? Add a date to each event, as far as you can remember. Write down as many events as you can fit into the line. Use a pencil so you can add events you remember later.
Now, you are ready for your Power Point. To make your introduction a bit interesting, mention something cool about yourself. This could be somehing about your personality or somehing you do regularly. Point out that you are this way because of the things that happened in your life so far.
For each slide, add points about one of the events on your timeline. I am sure you would be surprised at how much there is you can use. I bet you will not even use all the information you came up with.
I do not know how long your teacher wants this to be, but I hope these ideas helped a little. And on a final note, although school events are important, try to also include things that happened to you outside of school.
