Nurses are responsible for any number of things over the course of a patient's care, and normally have a much more direct and consistent role in the day to day monitoring and maintenance of a patient's condition. Administering medication, taking and monitoring vitals, responding to and maintaining records of patient feedback, even administering fluid IVs and aiding a patient to eliminate bodily wastes, all of which are very important to record and document as they speak directly to the health and progress of the patient. It is also important to maintain such meticulous records for insurance purposes and/or as part of a patient's long term medical file. Proper documentation also protects the nurse, doctor and hospital against lawsuits and, of course, is required by state and federal laws. It is an essential part of a nurse's duties on a daily basis.
