Could you answer a few short questions to help us improve our service?” This standard request for filling out a questionnaire can be made more personal with some clause like: “We want your experience here to always be pleasant, and we are attentive to your needs and wishes…” etc., but the longer the request, the less often a patron will take the time to fill it out. Perhaps a light-hearted tone and a multiple-choice questionnaire would get more responses: You dined here now—(or whatever) Was it: Atrocious?_____ Bearable?______ Mildly pleasant?_____ Epicurean?_______ Ecstatic?_____ What was worse?
