The narrator of the story is telling the reader that he had a neurotic compulsion related to the old man's blind eye. There is absolutely  no logical connection between the murder and the gaze of the old man's eyes.

This is a type of arabesque that Poe is famous for writing. He was able to delve deep into the inner workings of the deranged mind.  Poe was a writer of the macabre and detective story. He is able to take the reader deep into the mind of the madman.

The narrator of "The Tell-Tale Heart" had to have a motive for committing the crime. In his confession, he tells all to the police.
