The title refers to both his occupation as well as his life. The blues evolved out of slave work songs, which were meant to provide hope that some day would be brighter than the one happening right now. The fact that Sonny is playing Jazz and the blues has a two-fold effect on the title.
That Sonny is using drugs contributes to his depressive disposition and his blues felt throughout the book. Sonnys having been released and his use of the music of hope to overcome his situation provides an ironic use of the title to relate to both his life and his occupation.
