Eli Lilly Type 2 Diabetes Primary Care Award

Purpose: A $15,000 grant is available for the development, implementation, and evaluation of an innovative approach to the delivery of collaborative diabetes care and education to persons with Type 2 diabetes treated in a primary care setting. Proposed projects must include at least one physician in the primary care practice and at least one diabetes educator who is involved in all aspects of planning and implementation.
