Dimmesdale becomes increasingly ill and weak due to his sin (which isn't revealed until later in the book). Chillingworth also find out about the scar on Dimmesdale's chest that resembles the scarlet letter A that Hester is forced to wear. Hawthorne is manifesting spiritual trauma with physical trauma. Dimmesdale also becomes more vehement in his sermons about sin.
