The "two households, both alike in dignity" are not named in the prologue.  It is not until Sampson and Gregory, Capulet servants, are talking about how "a dog of the Montague house moves" them to fight that we learn the names of the feuding families: the Capulets and the Montagues.  It is "from the fatal loins of these two foes" that our "star crossed lovers", Romeo and Juliet, originate.

The two households are the Montagues and the Capulets--sworn enemies and parents to Romeo and Juliet, respectively.  This causes problems down the road since the two teens fall in love and realize their love is forbidden based on the feud between the their families which runs so deeply that even their servants fight one another in the streets of Padua.
