For one, the message that the Friar sends to Romeo in Mantua goes astray. Romeo does not know that Juliet is not truly dead--he only hears that she is which leads him to buy poison and head to the Capulet tomb.
For another, the nurse and the Friar had information that they should have stepped forward with when the plan for Juliet to marry Paris was introduced.
The Friar also marries the two behind their parents' backs and then, instead of telling their parents when Romeo is banished, he comes up with a plan that actually catapults them toward their deaths.
Lastly, the Friar leaves Juliet in the tomb with Romeo's body. He should have recognized the fragile mental state she was in and expected her to act irrationally.
