The above commentator rightly notes the major activities of the characters in ‘Macbeth’. Since the question is based on the important entrances and exits, it would be justifiable to segregate them in order of their occurrences.

    ENTRANCES:

The entrance of the three witches in Act I, Sc. i and in Act II, Sc. ii.
The entrance of Macbeth at the end of his “deed” in Act II, Sc. ii with his bloody hands.
The entrance / re-entrance of the “Ghost of Banquo” in Act III, Sc. iv.
The entrance of Lady Macbeth in Act V, Sc. i.
The entrance of Macduff in Act V, Sc. viii.

    EXITS:

The witches vanish in Act I, Sc. iii.
Macbeth leaves to murder Duncan at the end of Act II, Sc.i.
Macbeth and Lady Macbeth both “retire” to their “chamber” at the end of Act II, Sc. ii.
“Lady Macbeth is carried out” in Act II, Sc. iii.
The “ghost of Banquo” disappears several times in Act III, Sc.iv.
The exit of Lady Macbeth in Act V, Sc.i.
