Where did the holocaust take place; in what countries?

In general, the Holocaust happened in Germany and in Eastern Europe. The victims of the Holocaust were gathered from across Europe, but most of the actual killing was done in parts of Eastern Europe that had been taken over by the Germans during World War II.
For example, the best-known of the camps that were used to kill Holocaust victims were in Poland. These camps included Chelmno, Belzec, Sobibor, Treblinka, and the most notorious of all the death camps, Auschwitz. Auschwitz was the site of the deaths of more than a million of the victims of the Holocaust. These were the major death camps that were used in the Holocaust.
