The movie One Flew over the Cuckoo's Nest, starring Jack Nicholson is somewhat dated. Mental institutions operated on a different understanding of many mental disorders in the 1970s and 80s than we have now. Relating the movie to today would focus on how treatment for different disorders (e.g. megalomania, schizophrenia, Obsessive Compulsive disorder) has changed from the treatments in the movies to the treatments used today.
You should analyze how the different characters react to stimuli and situations, and how that differs from how you would react to the same thing. You should also make comparisons to Jack Nicholson's character; because his insanity is questionable, he was acquitted on an insanity defense, but was committed to the "Cuckoo's Nest" on the basis of his insanity. When I watched it, the plot development seemed to indicate that the staff, not the patients, were the insane ones.
