Marlow's biggest challenge has to do with the "changes on the inside". He was warned of these by the doctor in the Company's offices. He recognizes these changes occurring during his trek to the Central Station, and then at the Central Station, he throws himself into work because that's what shows a man's true worth to himself. This fight against this change culminates when Marlow is at the brink of killing Kurtz. Marlow recognizes that he is at the edge of a cliff that Kurtz has fallen off. In spite of this apparent victory, back in Europe, Marlow realizes he has changed when he cannot bear to tell the truth to Kurtz' fiance even though Marlow can't bear to tell a lie.
