I have never taken the stance that the narrator's mental state is deteriorating, personally. I found him to be quite sane and detailed...a concerned friend and one who will be there to be by his friend's side when he needs him. It could be said that near the end of the story when they lay Madeline to rest and she reappears that his state of mind could be a little suspect, but to me, this is not the case at all.
